We welcome your patient referrals. At the VMS, we encourage a team-oriented relationship with all North Texas veterinarians in the treatment and management of our clients and patients. Our doctors and staff will at all times promote pet health and welfare by providing quality diagnostic services in the care of pets presented to our hospital. The staff veterinarian who receives your patient will call to discuss the case with you as soon as possible within the daily case load.
Steps for Referring Veterinarians
1. Fax/Email
- Please click on the VMS Referral Form to submit the request online, or print it out and and fax to 682-223-9771.
- When possible, please include the patient’s most recent medical history and all copies of lab work completed. Digital x-rays may be emailed to info@vmsdfw.com.
2. Call
- Please call ahead, or instruct your client to fill out the New Patient & History Form, or call us directly to schedule an appointment.
- Please be prepared to fax or email us medical records and a completed VMS Referral Form prior to calling to schedule an appointment.
3. Radiographs
- It is recommended that you have the clients bring any radiographs that you have taken with them to their appointment. We will have the clients return these radiographs to you after their appointment.
- If you have digital radiography at your clinic, the client can bring a CD or flash drive with them to the appointment, or you can email the radiographs to info@vmsdfw.com. Please call our office if you have any questions regarding instructions and/or to confirm that we have received the patient’s radiographs.
